    Skydive Booking Form

    Skydivers Details

    The maximum weight restriction is 15st 7lb. The maximum height is 6ft 3". In addition, your weight should be in proportion to your height. ​
    If your height or weight exceed these limits, some airfields may still be able to accommodate you so please feel free to contact us to discuss further.

    You must be aged 16+ to do a tandem skydive.
    ​If you are aged 16 or 17 years, a parent/legal guardian must give explicit permission by completing the declaration section later in this form.

    Skydive Date

    Please provide a first and second choice of date. Most often you will be allocated your first choice but a second choice is required just in case your first choice of date is fully booked or not available.

    Skydive Airfield

    To do a charity skydive, you MUST raise a minimum of £350 in sponsorship. ​They way this is broken down varies by airfield.

    ​Your sponsorship is broken down in this way to ensure that your charity skydive doesn't cost you a penny (on the condition that you raise the minimum sponsorship amount). You may choose to pay the remaining balance yourself  and forward all the sponsorship you have raised to the charity, but this is entirely optional.

    Parent/Legal Guardian Declaration (under 18s only)

    If the skydiver is aged 16 or 17 years, permission to skydive must be explicitly given by a parent or legal guardian.
    We recommend that you (the parent/legal guardian) read all the information outlining the nature of the tandem skydive course that the young person in your care has chosen to undertake, in addition to the booking terms and conditions below. Please contact us if you have any further questions.
    By completing the section below, you confirm that you (the parent/legal guardian) have given consent for the student to participate, and that they fully understand the nature and risks of participating in this activity.

    Booking Terms and Conditions

    All charity skydivers MUST raise a minimum of £350 each in sponsorship to be permitted to skydive.

    ​​Terms and conditions of booking a charity tandem skydive:
    • The term ‘skydive’ applies to any type of parachute descent.
    • The airfield will make every effort to comply with the preferred dates for booking. If both the dates are unavailable, Lennox Children’s Cancer Fund, or the airfield, will contact you before proceeding with the booking to arrange another date.
    • Bookings are made on the understanding that any skydive will only take place in accordance with the safety limitations imposed by the Civil Aviation Authority (CAA) and the British Parachute Association (BPA).
    • A booking, once made, is final and the date cannot be changed with less than 21 days notice. An administration fee may be charged for changing the date once agreed.
    • A minimum of £350 must be raised in sponsorship before the skydive date. This will cover the non-refundable deposit, the remaining skydive balance and a donation for Lennox Children's Cancer Fund.
    • The balance of payment is due on or before the day of the course. Any fee(s) or deposit(s) paid are nonrefundable.
    • If you fail to turn up on the day of the course (irrespective of reason or cause), you will not be permitted to a refund of any fee(s) or deposit(s) paid or be permitted to commute any fees paid towards another course on another date. You will still be liable for the balance of payment.
    • If inclement weather or technical problems with the aircraft prevents skydiving on the day booked, you will be able to re-book a mutually convenient date free of charge. Refunds will not be made under these circumstances. The skydive must be re-booked with 12 months of the cancellation date.
    • Due to irregularities in weather, aircraft limitations and numbers of participants on the day of the course, you may need to be at the drop zone for the entire day, irrespective of your arrival time.
    • On the day of the course, if you do not conform to the age, weight and health requirements of the CAA and the BPA, or you do not have the correct paperwork required for skydiving (see below), you will not be permitted to take part in any skydive and the balance of payment will still be due.
     
    What you will need at the airfield:
    After booking, you will be emailed a confirmation of your tandem skydive and two sets of forms. You MUST complete these forms and take them with you on the day of your tandem skydive. Without them you will not be able to skydive.
    • BPA forms for medical information, declaration and student membership - a medical declaration
    • UK Parachuting Student membership form
    Don’t forget to also take:
    • The amount of money required to cover the cost of your skydive
    • The minimum amount of money required for the charity, or a print out/evidence of your online fundraising page if you raised any of your sponsors online
    • If you raised all of your sponsorship online, you must contact the charity approx two weeks before your skydive on 01708 734366 so that we can arrange an alternative payment method with the airfield.

    Insurance:
    Included in the price of your skydive is temporary membership to the BPA, which automatically covers you for third party liability up to £2,000,000. This does not however, cover you for personal injury and you are therefore advised to arrange special personal cover for yourself. We can give you information on personal injury cover on request. Please note that any cover you take out does not necessarily have to be through the companies we advise. If you are in any doubt at all about what is being offered to you, you should seek advice from your insurance broker.

    Non-Refundable Deposit

    The registration fee of £30 per person is due upon booking. This is non-refundable.
    We will call you when we receive your form to take payment by credit or debit card. If you would prefer to call us, you can reach us on 01708 734366.
    If paying by cheque, please make your cheque payable to 'UK PARACHUTE SERVICES LIMITED' and post it to: Lennox Children's Cancer Fund, 26 Hemmells, Laindon, Essex SS15 6ED. Your booking will not be made until the payment has been received.
